 Envelope for OMR Application Form
IMPORTANT :

The OMR Application Form should bear the seal of the Institution/MPPEB office, from where it is issued to the candidate, which confirms the payment of test fee. In the absence of the seal, the OMR Application Form shall be rejected. It is thus in the interest of the candidate to ensure that the OMR Application form bears the seal of the concerned institution

Clinical : Medicine including Dermatology and Venereology, Surgery, Obstetrics
& Gynaecology, Opthamology, ENT, Paediatrics, Orthopaedics, Anaesthesiology,
Radiodiagnosis and Radio-Therapy.
Non Clinical : Anatomy, Physiology, Pharmacology, Pathology, Preventive and
Social Medicine (PSM) and Forensic Medicine.

Highlights of Pre-P.G. – 2012
This test is conducted only for those who have already completed their M.B.B.S./
B.D.S. degree and desire admission to Post Graduate MD/MS, MDS Degree and
P.G. Diploma courses (Clinical and Non Clinical) in the Medical Colleges/Dental
College of Madhya Pradesh.
Examination City : Bhopal.

IMPORTANT INFORMATION
1. Eligibility :
Before filling the Application Form, the candidate should ascertain whether he/she fulfils all the requirements and is eligible for admission to MD/MS/Diploma/MDS Course. The conditions of eligibility are mentioned in Rule No. 15 of this Rule Book.
Candidate’s Responsibility : Candidates who appear in the test would do so at their own risk and responsibility so
far as their eligibility is concerned. Hence, candidates are advised to ascertain their
eligibility before applying.
2. Examination City : The test will be conducted at Bhopal Only.
3. Availability of Application Form : (a) The Application Forms for the test will be available from MPPEB counter and designated institutions on cash payment of Rs. 1500/- for (UR & OBC), 750- (SC & ST) (Cost of Application form includes the Test Fee) from 18.01.2012 to 04.02.2012.

4. Last date for submission of OMR Application Form : Duly filled OMR application form must be sent by registered post/speed post so as to reach MPPEB or submitted in person at MPPEB Office, Bhopal on or before 05:30 PM on 10.02.2012. MPPEB shall not be responsible for any postal or other delay in receipt of application form. Application forms received after due date and time will be summarily rejected.
5. In-Service candidates shall have to submit their OMR application forms as mentioned at S.No. 4 just like other candidates. They will also have to submit their In-Service form in prescribed proforma (Form-5 as given in the Rule Book) to the Director, Health Services, M.P., 4th floor, Satpura Bhawan, Bhopal.
